Health Outreach Mobile Engagement (H.O.M.E.) Program
The H.O.M.E Program provides a multi-disciplinary, multi-sectoral, mobile response to improve the health outcomes and health equity of highly marginalized individuals in London, Ontario who face barriers to accessing traditional models of care.
Types of Services Provided 2021-2022
Year 1 (January 2021 to January 2022) Metrics:
1059 unique clients accessed medical and wraparound care services
2723 client visits to the H.O.M.E. Program’s mobile supports
1175 non-essential emergency room visits were potentially prevented
780 chronic illness care visits were conducted
498 visits were focused on mental health care
182 visits were conducted to provide infectious disease care
84 individuals without a care provider were rostered to primary care
64 prenatal care visits were provided
18 emergency medical interventions were given
12 overdose responses were conducted
The H.O.M.E Program operated for a total of 231 days during the first year of program implementation.
Safer Opioid Supply Program
The main objective of the Safer Opioid Supply (SOS) program is to use a harm reduction approach to reduce some of the health risks associated with substance use, particularly overdose deaths related to fentanyl contamination within the unregulated opioid supply.

152 New Clients Enrolled in 2021. 248 Total Clients.
20,323 encounters by SOS clients with the health care team.
59.6% of SOS clients stated their physical health improved since starting Safer Supply
47.4% of SOS clients reported decreased involvement in criminal activities as a means to obtain substances since starting Safer Supply
